Registration Overview

Get ready for your first day of practice by ensuring you have the necessary paperwork in order.

Before reporting to practice, please remember to bring the following five items:

1) BVSD athletic permission slip

2) Activity Athletic Insurance Waiver

3) Parent Permission Form

4) CHSAA competitor brochure

5) BVSD Athletic Activities Policy Acknowledgment

These documents are essential for your participation in athletics and help maintain a safe and organized environment. Make sure to have them completed and ready to submit before starting your exciting journey as a student-athlete.

You will also need to provide your current Physical to the main office. If Kristine Ferfache has received an email from you with your physical, we have it on file. Please check if you are unsure. Failure to possess a current physical will result in the athlete being excluded from practice.

We encourage you to scan the completed forms attached and email them to kristine.ferfache@bvsd.org. This is the preferred method.

If you are unable to scan the forms, please bring hard copies to Kristine at Fairview HS.

Please note: Without the required registration paperwork and current sports physical, you will not be permitted to participate on the field. NO EXCEPTIONS.

To ensure participation on the first official day of practice, please complete the online registration by Thursday, Aug. 3, 2023.

Payments must be made during the registration process. Kristine Ferfache will provide the necessary information to guide you through the payment.

Payments are made through BVSD RevTrak system. Families are required to create a new account (refer to the FAQs for instructions). Visit the FHS RevTrak Website today to create your new account! Families without internet access at home can register using a school computer or obtain a physical copy of the athletic registration packet from the school athletic office during school hours. Cash or check payments can be made in person.

For any specific questions, please reach out to the Athletic Director or the Athletic Administrative Assistant at the school.

Sports Physical Requirements

In accordance with the guidelines set by the Colorado High School Activities Association (CHSAA), all student-athletes are required to undergo a Pre-participation Physical Evaluation (PPE) or a yearly sport physical. This evaluation is crucial in screening the athlete for any injuries or illnesses that could impact their participation. The sports physical must be conducted annually by a physician to ensure the safety of the student-athlete during athletic activities. Prior to participating in tryouts, practices, or competitions, the completed sports physical form must be submitted to the school's athletic office.

Please note that the standard sports physical form provided by the physician's office is acceptable. If you require a form, you can download and print the CHSAA Sports Physical Form or obtain one from the American Academy of Pediatrics Website.
