REQUESTING A TRANSCRIPT FROM FAIRVIEW HIGH SCHOOL
How to Request an Alumni Transcript

Please complete ALL requested information and be sure to print it clearly.

  1. Click here for the Fairview HSAlumni Transcript Request form.

  2. Take a photo or scan the completed request form. Email a copy of the completed request form to FHStranscripts@bvsd.org.

  3. Each transcript request costs $2. To make your online payment click here.https://bvsd.revtrak.net/high-schools/fhs/fhs-additional-items-and-events/#/v/fah-transcripts

  4. After your payment has been completed you will receive an email confirmation receipt. Please forward the receipt to FHStranscripts@bvsd.org

We must have an image of the completed alumni transcript request form (signed by the student) AND a copy of the receipt of payment to process the transcript request.

 

You can mail the request form AND cash/check to FHS if you don't have internet access to

Reunion tour requests between June 1st - August 15th require booking through Community Use.

  • Tours offered Monday-Friday at 5:00 PM (approx. 1 hour)
  • Saturdays at 11:00 AM

Please note: All community building reservations must be submitted to BVSD Community Use. Click here to learn about Community Use forms and information. BVSD Community Use does not accept Building Rental Requests after May 1 through July.
